Our Teams

GPs are joined by other health professionals - Nurse Practitioners, Occupational Therapists and Pharmacists - to form five multi-disciplinary ‘KeyTeams’ that will each take on responsibility for caring for a specific group of patients.

Team Alyn

Team Alyn provides face-to-face consultations from the Central Prestatyn site and the Meliden site. 

Led by Dr Sue Kidd, with fellow GPs Dr Fraser Campbell and Dr Paul Williams, they have a great team supporting them, allowing the GPs to spend more time with those patients with particularly complex medical needs. 

Lesley and Vicky are the Nurse Practitioners in Team Alyn, and are both able to prescribe independently, where required. 

Karen, the Pharmacist is on hand to help you with any prescription related matter.

Linda, the Occupational Therapist can help find non-medical solutions to improve your well-being and help understand what matters most to you.

Team Clwyd

Team Clwyd provide their face-to-face consultations from the Seabank site in Prestatyn, and the Rhuddlan Surgery site in Rhuddlan. They also provide a small number of consultations from the central Prestatyn site. 

Dr Ffion Williams and Dr David Bradshaw are the Team Clwyd GPs. Until Ffion starts with us in late summer 2016, we will have locum cover in place to support David.

Lesley and Esyllt (Es), and the nurse practitioners in the team, and are very well experienced. Along with Rob, the Pharmacist, and Lucy, the Occupational Therapist, they can address a huge range of problems and manage your ongoing care, without you needing to see the GPs.

Team Dee

Team Dee provide face-to-face consultations from the Central Prestatyn site and from Ffynnongroyw surgery. 

Led by Dr Clive Morrison, alongside fellow GP Dr Luke Pereira, the wider team includes Nurse Practitioners Maxine and Katie, Pharmacist Julia, and Occupational Therapist Mark. They are all experienced practitioners. 

If your needs relate to particularly complex medical problems then the GPs are there to help; for ongoing follow-up, or to deal with problems which fit with their areas of specialist expertise then the other team members are very happy to oversee your care.

Team Elwy

Team Elwy provides most of their face-to-face consultations from the Central Prestatyn site, though with a smaller number provided from the Seabank site too. 

GPs Dr Nick Shah and Dr Gill Breese are very keen to help you find simple ways to manage your health and well-being.

They are supported by Nurse Practitioner Sarah, by Pharmacist Becky, and by Occupational Therapist Alexis.

Team Brenig

Team Brenig is our 5th team, and has a focus upon patients in care homes, patients who are housebound with complex medical needs, and patients with advancing frailty. This ensures that those patients receive the right level of care in a timely way. 

Practice Nursing Service

Our Practice Nurse and Healthcare Assistant Team are there to help you with all of your primary care nursing and monitoring needs. The team supports patients from any KeyTeam.

Physiotherapy

The Healthy Prestatyn / Rhuddlan Iach service has two physiotherapists attached. They are important team members, supporting the whole service, and focus upon providing acute advice for musculoskeletal problems. Patients requiring more longterm physiotherapy are usually followed up by the area physio team in the Royal Alexandra hospital in Rhyl. 

The Support Team

Alongside the professionals above, is an army of support staff who are a vital part of the Healthy Prestatyn / Rhuddlan Team. They ensure that everything runs smoothly, both "front of house" (for example our receptionists and telephonists) and "behind the scenes" (for example our prescription clerks, records clerks, summarisers, and secretaries). All have an essential role to play!

The Leadership Team

The Leadership team steers the service on a day-to-day and week-to-week basis to ensure that we offer the highest possible level of service and to ensure that we continue to grow in the way in which we meet your needs.
